Patient: 32 years, female
Clinical situation: #26 recession and furcation evolvement grade | hopeless tooth
Surgical solution: -sinus lift
-extraction #26 + immediate implant placement
Restorative solution: Provisional: Immediate acrylic crown screwed retained
Final: Metal-ceramic crown
Surgery date: December 2011
- Patient complaint regarding #26: food impactation, gum recession, tooth mobility
Treatment Planning
When you are faced with the extraction of the 1st molar, you know that the residual bone will not be enough for a short implant placement and the patient does not want to be without teeth… (and she is your friend and is begging you…)
What would you do?
1st phase:
- Sinus lift in the region of #26 using xenograft material
2nd phase
- tooth extraction + implant placement and immediate acrylic crown
3rd phase
- Metalceramic crown
Surgical Procedure
First approach was the sinus lift with xenograft material. We like to use piezoelectric surgery for this proposed, because we can control better the osteotomy and it reduces the chances of membrane rupture.
- Extraction – odontossection in order to try to keep the remaining alveolar bone
- Immediate implant placement
